Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 12,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 60 locations to serve some of our community’s most vulnerable—and most resilient—citizens.

Job Summary:

Clinic Hours: M, W, F 8A-5P, T 8A-12N, 4-8, and Th 8A-8P
Our nurses practice a type of medicine unique in Boston. We are a nationally recognized leader in offering continuous care across a range medically complex cases with multiple co-morbidities. We are looking to hire a compassionate and dedicated nurse for our Primary Care/Urgent Care Clinic to provide a full range of nursing activities including direct care and nursing case management activities, immunization, and screening.
Responsibilities:
You’ll work as a member of the patients’ interdisciplinary care team, attending daily team huddles when in clinic, clinic staff meetings and team break- out sessions. Daily, you’ll provide education to patients regarding PCMH, conduct triage and nursing assessment of walk-in and scheduled patients and participate in the identification of, and care planning for, high risk/ enhanced care patients. Other responsibilities will include, but not be limited to:
- Providing quality monitoring, as directed, through record keeping and patient health indicator analysis
- Assisting provider access to primary and episodic care to patients from outreach sites.
- Providing access and patient care with BHCHP nurse liaisons.
Qualifications:
Ideally, you come to us with:
- A Commonwealth of Massachusetts RN license
- Previous experience in community health center or primary care experience
- Strong medical/surgical assessment skills
- Interest in public health, primary and preventive nursing/medicine
- Interest in working with under-served populations
- Understanding of substance abuse and mental health issues
Compensation:

- The compensation starts at $37.82 and increases based on years of experience.
