Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 30 locations to serve some of our community’s most vulnerable—and most resilient—citizens.

Job Summary:
Individuals who identify as transgender or gender diverse (TGD) are some of the most marginalized
members of our society, especially if they are experiencing homelessness, and they face worse health outcomes as compared with their cisgender counterparts. In response to this inequity, BHCHP developed a Transgender Program in 2008 to address the health care needs of the transgender and gender diverse community who experience homelessness or unstable housing in Boston.

The Program Coordinator will be responsible for a case management panel comprised of TGD patients
who have no other dedicated case management services. They will coordinate with other Case Managers as needed to assure all Transgender Team patients’ needs are being met, and that there is no duplication of services. This person will also do outreach and recruiting of patients, event management, foster empowerment including linkages to support groups, and participate in community advocacy efforts to decrease disparities in health care for this population.

Hours: 40 hours per week; some evenings required

Responsibilities:
  • Build relationships and collaborate with multidisciplinary teams of providers and nurses to provide and assure comprehensive case management services to homeless individuals.
    • work with clients to obtain the full range of benefits and services including benefits (SSI, General Relief), housing, primary care, social and psychiatric services, changing name and gender on legal documents, and all other necessary services; re-engage with TG patients out of care.
  • Co-lead (with the Americorps member) the planning for Transgender Day of Remembrance (TDOR) events for BHCHP, First Event, and TG health fairs; participate in community advocacy efforts that BHCHP is already a part of, such as the DPH TG Taskforce and the MA Trans Health Coalition, and others as appropriate; provide resources and advocate for inclusive practices at BHCHP and our community partners.
  • Collaborate with external organizations as needed to create support groups for BIPOC transgender clients in both English and Spanish and assist with facilitation; also co-facilitate existing BHCHP English support group.
  • With the Program Director, co-supervise the AmeriCorps member, and collaborate with the AmeriCorps member on a variety of projects, including outreach/recruitment and community event planning; assist with grant-writing as needed.
  • Present to external entities about the TG Program as needed, often along with a TG Program patient.

Qualifications:
  • BA/BS preferred, especially behavioral health or social services-related degree.
  • 3-6 years of case management and managed care experience; prior experience working in public health, social work, or community health.
  • Experience working with gender-diverse populations and demonstrated interest in working with an underserved population.
  • A firm understanding of homeless health care, transgender health, and Harm Reduction.
  • Excellent verbal and written English communication skills required, bilingual English and Spanish speaking required.
  • Problem solving skills required.

Compensation and Benefits:
  • The compensation starts at $49,500.00 annually and increases based on years of experience.
  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
