Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 30 locations to serve some of our community’s most vulnerable—and most resilient—citizens.

Job Summary
Hours:  full-time, Monday - Friday 8:30am-5:00pm


Currently, drug overdose is the leading cause of death among homeless individuals in Boston, and patients in our buprenorphine program have a two-thirds lower death rate in the months when they’re enrolled with us. We are expanding our work to include treatment for problems with cocaine and methamphetamines, and continuing to expand low-barrier access to treatment for opioid problems. We are hiring a Clinic Care Coordinator for our Recovery Team.

To do this job, you’ll need the ability to provide warm hospitality and welcoming; meticulous organization; a commitment to supporting people who use drugs where they are; and a belief in recovery as a process each person defines for themselves. You must also have a commitment to equity and inclusion, and an understanding of how to make daily operations inclusive and equitable.

Responsibilities

More specifically, the Clinic Care Coordinator will:
  • oversee the walk-in/ Open Access schedule and ensure that patients know where they are in this schedule and know the plan that the team is formulating to address their needs.
  • provide recovery navigation services to individuals in our program, helping them connect to both internal and external recovery supports and programs
  • manage intakes, data questionnaires, recovery rewards payments, and raffles and other prizes for people in our stimulant treatment program
  • provide backup for community engagement activities like educational lunches, health fairs, and other events
  • support team meetings including coordinating schedules, minutes, records of case reviews, and managing voicemail and fax inboxes.

Qualifications:
  • Demonstrated commitment to harm reduction, reducing stigma, and working together with people experiencing addiction or recovery; we especially encourage those with lived experience
  • Understanding of issues related to homelessness, poverty, and racism, and how these influence the lives of people living with substance use disorders.
  • Experience in administrative roles, including maintenance of databases or patient/customer lists; comfort with programs like Microsoft excel and ability to learn to use electronic medical records; ability to juggle different simultaneous tasks.
  • Experience or skills in welcoming or hospitality: and managing patient or customer flow, and helping patients or customers manage anxiety or frustration.
  • Experience in helping people navigate complex medical or social systems, for instance in case management, patient navigation, recovery support, or similar.
  • Ability to work with anxious, uncomfortable, or upset people and help them reach their goals; and ability to build long-term friendly relationships with customers or patients.
Compensation and Benefits:
  • The compensation starts at $22.07 per hour and increases with years of experience.
  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency, and the opportunity to work with local hospitals and community health centers.
