Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary: 
Hours: Full-time; Monday – Friday, 8:30am – 5:00pm

In keeping with our mission, we are seeking a talented and compassionate Outreach Care Coordinator. You will find challenging and satisfying work alongside colleagues who share your drive and your compassion. This position is a hybrid model, 3 days on site and 2 days remote.

In your daily practice, you will be based in the community and serve as a vital link connecting homeless individuals with serious mental illness, substance use disorders, and/or complex medical illnesses with needed care including mental health, substance use, and primary care as well as services that address social determinants of health needs. This work is a part of a MassHealth initiative that provides qualified MassHealth enrollees with a service benefit called Behavioral Health Community Partners (BH CP).  You will work as part of an interdisciplinary BH CP team and take responsibility for the preliminary outreach and intake of approximately 45 enrollees at a time. You will work closely with other care team members and will be responsible for engaging their enrollees mostly face to face and completing intake for BH CP Program. As the Outreach Care coordinator, you will then pass the assignment to a long-term Care Coordinator.
Responsibilities: 

  • Completes and documents primary outreach attempts to new assignments monthly including telephonic and In-person outreach to street, shelter, medical facilities, etc.; completes documentation in a timely, organized way. All patients on panel require at least once/month contact and documentation.
  • Completes intake paperwork including consents, comprehensive assessments, and care plans with enrollees within five months of assignment; completes initial, urgent case management need of patients such as transportation, re-scheduling appts, connecting with resources; connects patients to primary care.
  • Uses multiple Information Technology platforms to seek out information to locate and engage patients.
  • Triages and troubleshoot care need issues for enrollees; works with other team members to address needs and engage patients.
  • Participates in BH CP training and learning collaboratives.
  • Requests & send medical records to for care coordination purposes to providers, ACO, MCO as needed.
  • May provide intermittent, temporary coverage for other care coordinator panels as needed.
Qualifications:
  • Excellent customer service skills and the ability to communicate professionally with employees and enrollees both on the phone and in person.
  • Efficient, organized, detail oriented, and able to complete tasks in a time sensitive manner; self-directed with the ability to work both independently and as part of a team and interested in using data to help direct care.
  • Motivational Interviewing and Cultural Competency training a plus.
  • Experience working with homeless populations and/or populations with mental illness and substance use disorders; experience in case management or human services is preferred but not required.
  • Strong computer skills: Proficient with Microsoft Excel Spreadsheets, entering narrative and other data into a database, the use of the Internet to conduct information searches.
  • Bilingual English/Spanish preferred.
Compensation and Benefits:
  • The compensation starts at $22.07, and increases based on years of experience.

In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred added to your base hiring rate for demonstrated bilingual proficiency added to the base rate and the opportunity to work with local hospitals and community health centers.
