Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Boston Health Care for the Homeless Program (BHCHP) serves to provide the highest quality healthcare to our community’s most vulnerable and resilient patients. At the New England Center and Home for Veterans (NECHV), BHCHP operates a clinic that provides care for men and women who have served in our Nation’s Armed Forces. The clinic provides care within the emergency shelter, the efficiency apartments and the single room occupancies (SRO). The clinic provides integrated primary care, behavioral health care and urgent care as well as responds to medical emergencies within the Center to a population that often carries the burden of post-traumatic stress disorders (PTSD). Additionally, we run specialty clinics in the treatment of both Hepatitis C and Substance Use disorders.

As a Registered Nurse, you will provide nursing care for clinic patients as part of the multidisciplinary team.  This position is crucial to assure that clinically appropriate and timely decisions are made, and actions are taken around patient assessments, administering of medications, treatments and overall patient care. This role includes 32 hours on site at BHCHP's NECHV clinic and 8 hours at another BHCHP outpatient clinic site. 

Responsibilities:
 Direct Nursing Care
  • Direct nursing care to ensure that all needs of the patients are met, in conformance with established policies and procedures, as well as Department of Health rules and regulations
  • Provide all nursing related activities in association with primary care clinical operations at BHCHP primary clinic settings: including direct care and nursing case management activities, immunization and screenings
  • Provide triage and nursing assessment of walk-in and scheduled patients as directed; provide outreach to patients in their respective programs and housing outside of the clinic located at NECHV
  • Responsible for ensuring that the clinic is fully equipped and operational daily; assist patients with management of medications and understanding disease processes
  • Responsible for accurate assessments and documentation, reporting to appropriate personnel as indicated (i.e., on-call provider, nurse manager, Site Manager, etc.); responsible for administering medications and treatments to all the patients as assigned by providers as well as skillfully and correctly, documenting per policy.  Observe and report any adverse reactions
  • Responsible for updating and maintaining accurate treatment records, flow sheets and vaccine administration, documenting per policy; responsible for responding to medical emergencies within the NECHV, activating emergency medical services when indicated
  • Knowledge of, or willingness to become proficient in utilizing Harm Reduction Principles to provide non-judgmental clinical support for patients in all phases of recovery from substance use disorders; provides clinical support to case managers/care coordinators for complex care management for a cohort of patients including medication monitoring, review of intake assessments and care plan development
Communication:
  • Maintains a professional approach with confidentiality
  • Maintains the standards of accurate and complete recording and reporting ensuring that the clinical record for each patient is current and accurate
Education and Training:
  • Educate patients according to their needs, using nursing knowledge and skills, in such areas as TB, HIV, STIs and medications to promote their physical and mental well-being
  • Participate in orientation and in-service training for personnel, student nurses and other health professions as indicated
Secondary Duties and Responsibilities:
  • Support and attend mandatory in-service education programs as per facility protocol
Qualifications:
Education and Experience:
  • Commonwealth of Massachusetts RN license
  • Minimum of 2 years of nursing experience preferred
  • Experience with underserved population preferable
Compensation and Benefits:

  • The compensation starts at $40.00 per hour and increases based on years of experience.
